Basophils are the rarest white cells, involved in allergic and parasitic responses. They are so scarce that a low count is meaningless, and even a raised count is uncommon — when it does occur it usually travels with eosinophils and points towards parasites, allergy, or (classically) heartworm.

Basophils are the rarest of the circulating white cells — often absent altogether from a normal differential. They are close cousins of tissue mast cells, carrying granules rich in histamine and heparin, and they participate in allergic (hypersensitivity) reactions and parasitic responses. Because they are so scarce to begin with, the interpretive rules for basophils are unusually lopsided.

Reading a low result

A low basophil count is meaningless. Because the normal count is at or near zero, there is no "low" to detect — a differential reporting no basophils is entirely normal and carries no information whatsoever. This is worth stating plainly, because a zero on a report can look alarming when it is simply the expected finding.

Reading a high result

A raised basophil count (basophilia) is uncommon, and when it occurs it almost always travels alongside eosinophilia — the two cells respond to overlapping stimuli. The classic associations are:

  • Parasites, most notably heartworm in dogs, where basophilia with eosinophilia is a recognised pattern.
  • Allergic and hypersensitivity disease.
  • Mast cell tumours and, rarely, certain marrow disorders.
  • Occasionally, conditions with marked lipid abnormalities.

So a genuine basophilia is a modest but real prompt to think about parasites (heartworm in endemic areas), allergy, and — with other supporting findings — mast cell disease. It is interpreted with eosinophils rather than alone.

A practical caveat about counting

Basophils are notoriously difficult for automated analysers to identify reliably, precisely because they are so rare and can resemble other cells. A reported basophilia is therefore often confirmed on a blood smear by a human eye before it is acted upon. Treat an unexpected basophil count as something to verify rather than immediately interpret.

In chronic disease

Basophils have limited value as a monitoring marker given their rarity. Where a basophilia accompanies a diagnosed allergic or parasitic condition, it may settle as that condition is treated, but the eosinophil count is generally the more practical number to follow.

Basophils are rare in both species and interpreted the same way. The heartworm association is chiefly a canine consideration in endemic regions; allergic and mast cell associations apply to both. In each, a zero basophil count is normal, and a raised one warrants smear confirmation.

About the reference range

This page shows no range strip. Pawline draws a band only where it holds a verified reference interval, and for basophils the "range" is effectively zero to a very small number, which a strip would misrepresent. Read your result against the range printed on your own report, and note the absolute count rather than the percentage.

For orientation only, published figures capture just how small that number is: Cornell University (Advia 2120) reports absolute basophils of about 0–0.1 ×10⁹/L (dog) and 0–0.2 ×10⁹/L (cat), and IDEXX (July 2025) about 0–0.1 ×10⁹/L in both. Because the lower bound is zero, "none detected" is entirely normal and it is only a genuine rise that carries meaning. Your own report's range is what counts.

What to do next

A zero or low basophil count needs no action at all. A raised count is worth confirming on a smear and then interpreting alongside the eosinophil count — thinking about parasites (including heartworm where relevant), allergic disease, and, with other findings, mast cell disease. It is a supporting clue within the differential, never a stand-alone finding.
