Platelets are the tiny cell fragments that form clots to stop bleeding. A very low count raises a genuine bleeding risk and is taken seriously — but in cats especially, platelets often clump in the sample and make the reported number falsely low, so a low result is confirmed before it is believed.

Platelets (thrombocytes) are not whole cells but small fragments shed from large marrow cells, and their job is haemostasis — plugging breaks in blood vessels and kick-starting the clotting cascade. When a vessel is injured, platelets are first on the scene, sticking together to form a plug. Because that role is life-and-death, a genuinely low platelet count is one of the more urgent findings on a blood count.

There is, however, a crucial technical caveat that must be understood before any low count is acted on.

Reading a low result

A genuinely low platelet count (thrombocytopenia) matters because, below a certain level, the blood can no longer clot reliably and spontaneous bleeding becomes a risk — small skin or gum bruises (petechiae) are a classic warning sign. Real causes include immune-mediated destruction (the platelets are attacked and removed), consumption (used up in widespread clotting), reduced marrow production, and certain infections.

But there is a common false alarm: platelet clumping. Platelets are sticky and readily clump together in the collection tube, and an automated analyser counts a clump as a single platelet — so the reported number reads falsely low. This is especially frequent in cats, whose platelets clump very readily. For this reason, a low automated platelet count is routinely confirmed on a blood smear, where clumps and the true platelet density can be seen directly. A "low" count that is really just clumping is reassuringly common; a confirmed genuine low count is important.

Reading a high result

A high platelet count (thrombocytosis) is often reactive — a response to inflammation, recent bleeding, iron deficiency, or certain other conditions — and is usually less concerning than a low one. Rarely, a markedly high count reflects a primary marrow disorder. It is interpreted in the context of the rest of the blood count and the clinical picture.

In chronic disease

Platelet counts are tracked to monitor immune-mediated and consumptive conditions and their treatment, and to watch the marrow effects of some diseases and drugs. A recovering platelet count in a pet treated for immune-mediated destruction is a key sign of response; a falling count can flag consumption or marrow suppression that needs attention.

The interpretation is the same in both species, but the clumping caveat looms much larger in cats — feline platelet clumping is so common that a low automated feline platelet count is treated as unconfirmed until a smear is checked. In both species, the mean platelet volume (MPV) and the smear add context about whether the marrow is actively producing new platelets.

How it's measured

Platelets are counted by the analyser as part of the CBC, reported in ×10³/µL (or ×10⁹/L), and a blood smear is examined whenever the count is low to check for clumping and estimate the true number. Gentle, prompt sample handling reduces clumping. Read the value against your own report's range — and, if it is low, expect it to be confirmed on a smear before conclusions are drawn.

What to do next

A genuinely low, confirmed platelet count — especially with any bruising or bleeding — is taken seriously and worked up promptly for its cause. But a low automated count, particularly in a cat, is first confirmed on a smear, because clumping is such a frequent and benign explanation. Your vet will make that distinction before acting.
