Granulocytes are the front-line infection-fighting white cells — mostly neutrophils, plus eosinophils and basophils. Some analysers report them as a single group; the absolute count is the meaningful figure, and it's interpreted much like the neutrophil count it largely represents.

"Granulocytes" is an umbrella term for the white cells that carry visible granules in their cytoplasm: the neutrophils (by far the most numerous), the eosinophils, and the basophils. Some blood analysers — particularly compact in-clinic machines — report these together as a single granulocyte figure rather than splitting them out, so this is a number you may see when a full five-part differential isn't available.

Because neutrophils dominate the group, the granulocyte count behaves in most situations like a neutrophil count, and is interpreted along the same lines.

Reading a high result

A raised granulocyte count usually reflects the same drivers as a neutrophilia: inflammation, bacterial infection, and the stress/steroid response. If eosinophils are also elevated within the group, parasites or allergic/inflammatory conditions come into play. Because the group is a blend, a markedly raised granulocyte count on a basic analyser is often followed up with a full differential and a blood smear to see which cell type is actually driving the change — and, importantly, to look for the immature neutrophils (a "left shift") that distinguish genuine, active inflammation from a simple stress response.

Reading a low result

A low granulocyte count largely reflects a low neutrophil count (neutropenia), which can matter because it may indicate cells being consumed faster than they are made (overwhelming infection) or not being produced (marrow suppression). A low granulocyte count in an unwell pet is usually clarified with a full differential and, often, a smear.

In chronic disease

Tracked over time, the granulocyte count follows the intensity of inflammatory and infectious processes and the marrow's response, much as the neutrophil count does. Where more detail is needed — to separate the cell types, or to spot a left shift — a full differential is the better tool, so the granulocyte figure is best seen as a useful summary that sometimes needs unpacking.

Granulocytes are interpreted the same way in both species, with the same neutrophil-dominated behaviour and the same stress/excitement caveats that apply to the differential generally. In each, when the grouped count is notably abnormal, the next step is usually a full five-part differential and a smear to identify the responsible cell type and check for a left shift.

How it's measured

Granulocytes are reported as a percentage and an absolute count (×10³/µL), most often by analysers that give a three-part rather than five-part differential; the absolute figure is the meaningful one. A laboratory CBC with a blood smear provides the fuller breakdown into neutrophils, eosinophils and basophils. Read the value against your own report's ranges.

What to do next

Treat the granulocyte count much as you would the neutrophil count — a raised value points to inflammation, infection or a stress response, a low value to consumption or marrow suppression. When it is clearly abnormal, the useful next step is a full differential and smear with your vet, to identify which granulocyte is responsible and to look for the immature cells that signal active inflammation.
