Lymphocytes are the arm of the immune system that handles longer-term and viral responses and makes antibodies. Stress and steroids lower them, so context matters — and a persistent, marked rise is one of the flags that warrants a closer look for a lymphoid disorder.

Lymphocytes are the "adaptive" immune cells — the ones responsible for targeted, memory-based defence: coordinating responses to viruses, and (as B-lymphocytes mature into plasma cells) producing antibodies. On the white cell count they are usually the second-largest population after the neutrophils, and they are reported both as a percentage of white cells and, more usefully, as an absolute number.

Lymphocytes drive longer-term and viral immune defence and make antibodies. Stress lowers them; a marked, persistent rise warrants a closer look.

A quick note on percentage versus absolute: a change in the percentage can be caused entirely by other cell types moving, so it is the absolute lymphocyte count that is interpreted clinically.

Reading a low result

A low lymphocyte count (lymphopenia) is common and, importantly, is often not a sign of primary immune disease. The leading cause is the body's stress response: cortisol — whether from the stress of illness and handling (the "stress leukogram") or from steroid medication — drives lymphocytes down. So a mild lymphopenia in a sick or anxious pet is frequently just that stress effect. Other causes include acute viral infection and loss of lymph fluid (as in some gut diseases). Context — is the pet stressed, ill, on steroids? — usually explains it.

Reading a high result

A high lymphocyte count (lymphocytosis) has a few explanations, and the distinction matters:

  • Excitement/adrenaline, especially in young animals and cats — a transient, benign rise.
  • Chronic immune stimulation — a longer-running response to some infections.
  • Addison's disease — a lack of the stress hormone can leave lymphocytes inappropriately normal or high in a sick animal (the absence of a stress leukogram is itself a clue).
  • A lymphoid disorder — a persistent, marked lymphocytosis, particularly with abnormal-looking cells on the smear, raises the question of lymphocytic leukaemia and warrants further testing.

So while most high counts are reactive, a sustained and pronounced one is a flag that earns a closer look.

In chronic disease

Tracked over time, the lymphocyte count contributes to the pattern that distinguishes stress and reactive changes from an evolving lymphoid problem. A persistent, climbing lymphocytosis is followed up; a stress-related low count resolves as the underlying illness settles. As always, it is read within the whole differential rather than alone.

Cat vs dog

Lymphocytes behave similarly in both species. The excitement lymphocytosis is especially notable in cats and young dogs, so a modestly high count in a lively young patient is interpreted with that in mind. The stress-driven lymphopenia is common to both. In each, a marked and persistent lymphocytosis with abnormal cells is the finding that prompts investigation for a lymphoid disorder.

How it's measured

Lymphocytes are counted within the CBC differential, reported as a percentage and an absolute count (×10³/µL); the absolute figure is the meaningful one. A blood smear allows abnormal or reactive lymphocytes to be identified. Read the value against your own report's ranges, alongside the rest of the differential.

What to do next

A mild lymphopenia in a stressed or ill pet usually needs no action beyond addressing the underlying cause. A marked, persistent lymphocytosis — especially with atypical cells — is the pattern that prompts your vet to look further for a lymphoid condition. Because stress and excitement move lymphocytes so readily, the count is always read in context and over time.
