This marker has no established reference range in dogs or cats yet, so it is read as a trend rather than measured against a threshold.
Osmolality is the total concentration of dissolved particles in blood. On most panels it is not measured at all — it is calculated from the sodium, glucose and urea already on the same report, so it carries no information those three do not, and it is best read by looking at them.
Osmolality describes how concentrated the blood is: the total number of dissolved particles per kilogram of water. Body water moves toward the more concentrated side of a membrane, so osmolality is closely tied to thirst, urine concentration and how water is distributed between the blood and the tissues.

Most chemistry analysers do not measure osmolality. They calculate it from results already on the panel — principally sodium, with smaller contributions from glucose and urea (eClinpath).
That has a direct consequence for how it should be read: a calculated osmolality cannot tell you anything the sodium, glucose and urea did not already say. If it is high, one of those three is high, and that is the finding. This is why Pawline shows it as a trend without a reference band and never lets it affect the Healthspan Score — scoring it would count the same sodium, glucose and urea twice.
A raised calculated osmolality points back to its ingredients. In practice that usually means a raised sodium (most often a water deficit — dehydration, or losses not being replaced), a raised glucose, or a raised urea. Look at which of the three moved rather than at the osmolality itself.
A low value most often reflects a low sodium, which is more usually about excess water relative to salt than about salt loss alone (eClinpath).
There is one situation where measuring osmolality directly adds something a calculation cannot. Some substances are osmotically active but are not part of the calculation — they raise the measured value while leaving the calculated one unchanged. The difference between the two is called the osmolal gap, and a wide gap is used in emergency practice as a pointer toward certain toxins, ethylene glycol (antifreeze) being the one usually named (eClinpath).
This requires a laboratory to measure osmolality on an osmometer. It is a time-critical emergency investigation rather than a routine screening test, and a calculated figure on a wellness panel cannot substitute for it.
The physiology and the arithmetic are the same in both. The reference figures quoted for measured osmolality differ slightly between species and between laboratories, which is another reason a calculated value on a routine panel is read qualitatively rather than against a fixed cut-off.
Osmolality is not followed as a monitoring parameter in its own right. In chronic kidney disease, diabetes and disorders of water balance, the individual contributors — sodium, glucose, urea, and urine concentration — are what get tracked.
