The NLR divides neutrophils by lymphocytes to capture the balance between the immune system's front line and its longer-term arm. It's a cheap, emerging marker of systemic inflammation and stress that has shown prognostic promise — but it is highly non-specific, and a routine stress response raises it just as readily as disease.
The neutrophil:lymphocyte ratio takes two numbers already sitting on every blood count — neutrophils and lymphocytes — and divides one by the other. Its appeal is that these two populations tend to move in opposite directions under systemic stress and inflammation: neutrophils rise while lymphocytes fall. Combining them therefore amplifies a signal that either alone might understate, at no extra cost.

It has attracted genuine research interest in veterinary medicine as a cheap, universally-available index of systemic inflammation, with studies exploring its prognostic value across a range of conditions.
A high NLR indicates a relative shift towards neutrophils and away from lymphocytes — the pattern of systemic inflammation or a stress response. Research has associated higher ratios with more severe disease and poorer outcomes in various settings, which is why it is discussed as a prognostic index.
But the interpretive caution here is substantial and must not be glossed over: the classic "stress leukogram" — cortisol from illness, anxiety, restraint, or steroid medication — produces exactly this pattern (neutrophils up, lymphocytes down) without any inflammatory disease at all. A frightened cat in a waiting room can generate a raised NLR. Steroid treatment does it reliably. So the ratio is genuinely non-specific, and a single raised value in a stressed animal means very little.
A normal NLR is unremarkable. A low ratio (relatively more lymphocytes) can accompany the excitement response in young animals and cats, or — in a genuinely sick animal that has failed to mount the expected stress pattern — can be one of the small clues that raises Addison's disease, the same logic that makes an unexpected eosinophil count informative.
The NLR is an emerging index rather than an established clinical test in dogs and cats. There is no agreed reference interval or validated cut-off, proposed thresholds vary between studies and conditions, and much of the evidence is retrospective. Treating it as a threshold-based finding would over-claim considerably. Its honest use today is as a trend within an individual patient — an NLR falling as an illness resolves is more meaningful than any single number — and as a supporting observation alongside validated inflammatory markers such as CRP (dogs) and SAA (cats), which are purpose-built for the job.
Followed serially in one patient, the NLR can add texture to how systemic inflammation is behaving over time, and it costs nothing extra since both inputs are already measured. But where inflammation genuinely needs quantifying and tracking, the acute-phase proteins remain the better instrument.
The ratio is calculated identically in both species, and the stress-leukogram confound applies to each — arguably even more so in cats, whose excitement and stress responses can be pronounced. Neither species has an agreed clinical cut-off. In both, the ratio is read alongside the full differential rather than extracted from it.

Treat a raised NLR as a soft signal of systemic inflammation or stress, not a finding in itself — and ask first whether stress, illness or steroid medication explains it. If inflammation genuinely needs measuring and tracking, discuss CRP (dogs) or SAA (cats) with your vet, which are validated for exactly that purpose. As a free by-product of a test you already have, the NLR is worth noticing; it is not worth acting on alone.
